Fibrotic diseases are a major global health issue, affecting a large number of individuals. Our understanding of the pathogenesis of these fibrotic processes is incomplete, with significant heterogeneity in their etiology and clinical manifestations. There is a lack of appropriate and well-validated biomarkers, and most importantly, there is currently a deficiency of effective drugs to treat these diseases. Fibrotic diseases encompass a wide range of clinical entities, including systemic fibrotic diseases such as systemic sclerosis (SSc), scleroderma graft-versus-host disease, nephrogenic systemic fibrosis, and numerous organ-specific diseases, including radiation-induced fibrosis and fibrosis of the heart, lungs, liver, and kidneys. Despite the diverse pathogenic mechanisms underlying these diseases and the fact that they remain elusive in certain cases, they share a common characteristic: the uncontrolled and progressive accumulation of fibrous tissue in the affected organs, leading to dysfunction and eventual failure. Though there is significant heterogeneity in the pathogenic mechanisms and clinical manifestations that lead to the development of fibrotic diseases, many studies have shown that activated myofibroblasts are the common cellular component that ultimately results in the replacement of normal tissue with non-functional fibrotic tissue. Key signaling cascades, primarily initiated by transforming growth factor-β (TGF-β), involve numerous cytokines and signaling molecules that stimulate myofibroblast pro-fibrotic responses, providing potential therapeutic targets. Here, we briefly review the current knowledge regarding the molecular mechanisms of tissue fibrosis development and highlight some of the most critical challenges in researching fibrotic diseases and developing effective treatments, which often belong to a group of life-threatening conditions. Efforts to further elucidate the complex pathogenic mechanisms of the fibrotic process should be encouraged to achieve the elusive goal of developing effective therapies for these severe, untreatable, and often fatal diseases.
